Replay B keeps four refused requests with different batch sizes and reconciles using exact `refused_points`. Both replays reconcile independently at the request and point levels. Static tests reject request counters in point equations and point counters in request equations.

Review PR 309 and approve it if that diff and regression evidence establishes exact, dimensionally separate request and point accounting and closes the standing block.
